How do I know if I may qualify?

As of November 21, 1996 you (or your deceased loved one) must have :

• Been a Resident of Florida
• Had a History of regular cigarette smoking
• Documented Symptoms of a smoking related illness.

Qualifying illnesses include:

• Emphysema/COPD
• Bladder Cancer
• Cervical Cancer
• Throat Cancer
• Kidney Cancer
• Lung Cancer
• Oral/Tongue Cancer
• Pancreatic Cancer
• Stomach Cancer
• Stroke
• Heart Attack
• PVD/Buerger’s
